Unfortunately, the survey keeps insisting I answer the question about whether I work near where I live, despite the fact that I HAVE answered the question.

I’m having the same issue. In spite of my adding a check mark to question 5, I am getting the error message, “This question requires an answer.” with question 5 having an asterisk next to it. Hoping you’ll be able to fix this issue as I’m interested.

I have tried 5 times to fill in the form and it simply will not let me past Q5 – even though I *HAVE answered it, it says that I have to answer Q5!
Also the form, if I may say so, seems awkwardly constructed (not to mention US-centric) as I teach ages 2 to 18 yet the questions seem to let you only selct ONE age group from a pull-down list.
